Fracture of the femoral head is a severe, relatively uncommon injury; typically, it may have associated with traumatic posterior dislocation hip joint or without acetabulum fracture. The Pipkin classification most commonly used system. Presenting case report 28 years old young male motor vehicle road traffic accident suffered femur fracture on right wall Judet-Letournel type 4 Pipkin’s This patient was undergone for emergency intervention open reduction, fixation through Ganz insertion Herbert screws interfragmentary screw trochanteric osteotomy. followed up 1 year complete range motion at painless daily lifestyle. Specially treated better risk AVN head. Complications subsequent treatment osteonecrosis, post-traumatic osteoarthritis, heterotopic ossification implant failure.
